You can expedite your visit by filling out the "Patient Registration Form" by clicking on "Patient Forms" from the menu on the left.

If you wear glasses or contacts, please bring them with you. If you have a written copy or the packaging that the contacts came in, we request that you bring that with you as well. Please DO wear your contacts into the office on the day of your appointment.

Same day appointments are available upon request. Please ask the receptionist if we can accommodate you.
